In order to attend camp we have 3 forms that will need to be completed. Read the description and click to download each form below. You may fill them out completely online (including signatures) or you may print them out.

Form Turn-In
Forms may be submitted in person or online.
The Marching Band Reps will be collecting forms in the WLN Commons from 5:30 - 8 pm on Wednesday July 12th OR
This form is required by Camp Ohiyesa. The Band Reps must turn in these completed forms to YMCA prior to our arrival. Be sure to fill out completely as they do check and make sure you have your students date of their last tetanus shot (DTaP), as this is required. This form also requires student and parent signatures in 3 spots.
For Away Camp, we require a list of all prescribed medications. Please fill this form out completely with the medications/times/doses that your student needs to take medications while at Camp Ohiyesa.
If your student does NOT take any medications, you do not need to fill out this form. On your Parent and Student Information form in Membership Toolkit, you have indicated over the counter medications such as Ibuprofen, Tylenol, etc., that we are allowed to give your child when requested. We provide these so you do not need to send any medications with your child and we will double check your form before giving your child any over the counters.
If you know medications will change between July 12th and July 31st, please contact Amy at WLNMBcampforms@gmail.com to let her know. Otherwise, please turn-in your form on time as a medical book is created in advance and the medication line at drop-off is typically the longest wait due to families not filling out their form ahead of time.
Please pay attention to detail when filling out forms and double check before submitting. Make sure you have signed everywhere a form needs to be signed, all information is filled out, etc. With almost 200 students, it takes a lot of time for a Band Rep volunteer to review each form and then to have to notify a family if a form isn't fully completed.

Physical, Criminal History and Rules & Regulations
If you haven't turned in these forms you can do so by uploading into your Membership Toolkit account in the 2023 MB Registration Forms Turn-in.
Physicals are due July 12th. Your student must have the 2 page physical completed in order to take the field. Both pages needed to be completely filled out. Many are missing signatures. I will be sending an email out prior to the first practice letting anyone know if they are missing signatures to come in and sign on July 12th.
